Are slashes ("/" or "\") valid characters in supplemental photo names?

Users may be able to submit documents but the attachments do not come through in the output.  This can happen by putting a slash or other other special characters in the photo name.

To fix this, Delete the supplemental photo and create a new one choosing the correct photo from the tablet's camera roll.
